What to Bring
For your first appointment, please bring:

Most forms can be completed in advance online, so you’ll spend less time with paperwork and more time focusing on your care.
Working With Other Providers
Your oral health is closely connected to your overall health. When needed, we collaborate with dentists, specialists, and other health professionals to ensure you receive complete, personalized care. If additional treatment is recommended, we’ll guide you in finding the right provider and help coordinate your care, so nothing falls through the cracks.
